All applications for examination
shall be filed with the board on the form provided. When any person, or
persons, is designated and authorized to be or act as an agent for the
applicant, such authorization shall be in writing, signed by the applicant and
the person designated, a certified copy of which shall be filed with the
board. All applications shall expire and be cancelled after a period of one
year if the applicant fails to appear for examination within such period.
