(1) All certificates of
qualification to practice dentistry or dental hygiene, and all annual licenses
shall be issued by the board in the name of the board, with the seal attached.
(2) The board shall keep a record of all applicants for licensure to
qualify as a dentist or dental hygienist, of applicants rejected on
application or examination with the reason for rejection, of certificates of
qualification and of annual licenses issued, and of dentists and dental
hygienists.
